The mechanism
How it works.
Calcitonin is a peptide hormone, marketed mostly as the salmon version because it is more potent in humans than the native form. It binds calcitonin receptors on osteoclasts, the cells that break down bone, and quiets their activity so less calcium is released from the skeleton, while also reducing how much calcium the kidney reabsorbs. Both actions pull calcium out of the blood, so it is used in conditions of high bone turnover such as Paget's disease and in states of high blood calcium.
Safety & regulatory boundary

A pooled analysis suggested a possible increase in cancer risk with long-term calcitonin use, leading regulators including the EMA to restrict nasal calcitonin and advise against long-term osteoporosis use. It remains used for some bone and calcium conditions. Approved products such as Miacalcin are not the same as compounded or research-labeled calcitonin, and the approved uses do not extend to general wellness claims.
