The mechanism
How it works.
MDMA (midomafetamine) is a substituted amphetamine that forces the brain's serotonin transporters to run in reverse, flooding synapses with serotonin and also releasing dopamine, oxytocin, and norepinephrine. The resulting state of lowered fear and increased trust and openness is the point: in the studied protocol the drug is given during structured psychotherapy so a person can revisit traumatic memories without being overwhelmed, with drug and therapy meant to act together. This model is investigational, the Phase 3 results are contested, and the FDA has not approved it.

MDMA-assisted therapy is investigational and was not approved by the FDA, which asked for more study. MDMA is a controlled substance with cardiovascular, psychological, and abuse risks, and the studied model uses the drug only within supervised therapy. Trial results do not support recreational or unsupervised use, and underground or at-home use is not the studied treatment.
