Whether a drug that reverses sleepiness improves cognition in someone who is well rested.
- The signal
- Approved indications are solid: excessive sleepiness associated with narcolepsy, obstructive sleep apnoea and shift work disorder, supported by labelled products and a substantial trial programme. The cognitive-enhancement literature in healthy people is real but more modest than the reputation: reviews find consistent effects on attention, executive function and fatigue resistance, with the largest gains in sleep-deprived subjects and in those starting from lower baseline performance, and less impressive results on complex tasks in well-rested high performers. Registered trials continue across psychiatric and neurological fatigue contexts.

- Whether long-term use in healthy people has costs that short trials cannot see, particularly for sleep architecture and for the dependence question, which its scheduling reflects but the literature has not fully settled.
- Our read
- The evidence supports a narrower claim than the reputation: it reliably counteracts sleepiness, and it improves cognition most in people who are impaired to begin with. Buying wakefulness is not the same as buying intelligence, and the contraceptive interaction alone means this should not be treated as a benign nootropic.
The mechanism
How it works.
Modafinil's primary well-established action is inhibition of the dopamine transporter, which raises extracellular dopamine, particularly in regions that regulate arousal. It also increases histamine, noradrenaline and orexin signalling in wake-promoting hypothalamic circuits, which is why its wakefulness effect is more selective than that of classical stimulants and why it produces less peripheral sympathetic activation than amphetamines. That profile explains the pattern in the evidence. Where performance is limited by sleepiness, restoring arousal restores function, and the measured gains are large. Where someone is already alert and performing well, there is much less to restore, which is why effect sizes shrink in well-rested subjects on demanding tasks. The drug reduces the perception of effort and fatigue reliably, and that is not the same thing as improving the underlying capacity.
Safety & regulatory boundary
The consequential part.
Approved for specific sleep disorders; cognitive enhancement in healthy people is off-label. A Schedule IV controlled substance in the United States. Labelling carries warnings about serious rash including Stevens-Johnson syndrome and toxic epidermal necrolysis, drug reaction with eosinophilia and systemic symptoms, angioedema and psychiatric adverse events. It induces CYP3A4, which reduces the effectiveness of hormonal contraceptives - a clinically significant interaction that is routinely missed. Prohibited in sport in competition.
