The mechanism
How it works.
Plecanatide is a synthetic peptide built to closely resemble uroguanylin, a natural gut hormone. It activates guanylate cyclase-C receptors on the intestinal lining, raising cGMP and driving chloride, bicarbonate, and water into the gut so transit speeds up. Unlike the related linaclotide, it is engineered to be most active in the slightly acidic upper small intestine and to taper off as the gut becomes more alkaline, which is the design rationale for its tolerability profile. It is minimally absorbed and acts almost entirely within the intestine.

Plecanatide carries a boxed warning and is contraindicated in patients under 6 years of age, with avoidance in those 6 to less than 18, because of a risk of serious dehydration shown in young animals. It is approved only for specific gastrointestinal indications. Trulance is the approved product and is not the same as compounded or research-labeled plecanatide, and the approved use does not extend to general wellness claims.
